How to Access the Patient Portal:

Request Access: Call us at (302) 722-6550 or let us know during your visit to the office.

Receive Your Link: We will send a link to your email with instructions for initial login.
3.
Log In: Follow the instructions in the email to log in. After your first login, you can access the Patient Portal anytime via our website.
- Important: Preventive Primary Care utilizes the same Cerner System as our sister Practices, Alfieri Cardiology and Delaware Advanced Vein Center. If you already have an account, you can log in.
